eIAC – Intensive Ambulatory Care Program
The eIAC Intensive Ambulatory Care Program has been developed to support patients suffering from complex and multiple chronic diseases in managing their condition at home. It enables all people involved in the process of taking care of a patient to identify and address the root causes of the patient’s frequent admissions. Moreover, it includes a holistic patient assessment, teleconsultation and remote monitoring, as well as coaching options. By creating a cohesive system of care, this program is intended to help reduce hospital admissions, while providing the highest level of care to chronically ill patients.
